Student Admissions Requirements:

 

- All completed Registration Forms found in the Applications Packet.

 

- A copy of the Social Security Card (both Student and Parent(s).

 

- A copy of students Birth Certificate.

 

- A copy of Driver's License, Non-Driver's License or Military Identification of all authorized

   persons listed for pick-up.

 

- The student's original and up to date Immunization Form (Blue Form).

 

- The student's completed Medical Report.
